If all measures against said common imperfection (or to help detect it) were taken and it still happened, I'd deciddedly consider that as 'unexpected'.
If it was easily traced to a lack of care, it would be less interesting, but it may fit another session of the symposium, like manufacture related.
Here's a list of the various sessions so you can see where it would best fit (and if you don't know, the organizers will put your paper in the session it will be the best fit on the base of the abstract, so be as descriptive as you can).

The Failure Analysis Society is planning a session on 'strange and unexpected failure' for the next IMAT.
We consider a 'strange or unexpected failure' any event that should not have taken place but has, unexpectedly, or something with an outcome completely different from the one that was predicted, and for this specific session the scope could be much broader than materials-related failures.
I'm pretty sure some members of the ASM community that are not part of FAS have encountered that kind of events in their professional endeavours so if you think you've got something interesting, please say it here, submit and abstract or contact Erhan Ulvan or myself.
