    Agenda - Guest Speaker, Frank Zakar of the National Transportation Safety Board speaking on the Natural Gas Pipeline Rupture in Coolidge, Arizona (2021) April 8th, 6:00 PM to 8:00 PM at Arizona State University. See details below. Abstract: On August 15, 2021, a 30-inch-diameter natural gas transmission pipeline owned by Kinder Morgan, Inc, ruptured in Coolidge, Arizona. The rupture resulted in release of gas vapor that ignited and caused an explosion, destroying a farmhouse about 541 feet away, killing two of three occupants and seriously injuring the other. A 47-foot segment of the pipe was ejected from the pipeline leaving a crater in the ground. The pipeline was coated with two layers of spiral wrap tape as the primary means of corrosion protection and supplemented with an impressed current cathodic protection system. The NTSB determined that the accident was caused by tented tape wrap leading to stress corrosion cracking, a fracture at a longitudinal seam weld, and subsequent rupture of the pipe. Contributing to the rupture was Kinder Morgan’s failure to record the correct coating type used for this segment of pipeline, leading to a risk assessment that did not fully identify the risk of stress corrosion cracking. The program will define the function of the NTSB and provide detailed information of the metallurgical examination. Speaker: Frank Zakar is a Senior Metallurgist at the Materials Laboratory Division of the National Transportation Safety Board, Washington, D.C., and has been working at this agency since 1988. He performs numerous failure analysis cases per year, all of which some time or another, covered all modes of transportation investigated by the Safety Board. Mr. Zakar received his Bachelor of Science degree in Metallurgical Engineering from Polytechnic Institute of New York (merged into New York University), and Master of Science degree in Metallurgy from Stevens Institute of Technology. Prior to his career with the Safety Board, he worked for Lockheed Martin Corporation, as a metallurgical engineer, where he diagnosed material failures related to aircraft, surface and marine vehicles. **Location - Arizona State University Materials Engineering Lab, See detail map for parking and campus location.
