Famous Engineering Failures and How to Learn from Them

Abstract
Many engineering failures happen due to 1) greed 2) human negligence 3) lack of communication 4) unexpected behavior or mode of failure. In today’s talk we shall review some well-known (and a few lesser known) engineering failures. The technical aspects will be reviewed along with the associated ethics and/or decision-making that were associated with the failure.
Erik's Bio
Upon finishing his graduate studies at the University of Florida (2006), Erik Knudsen accepted a mechanical engineering position at Northrop Grumman in 2007. After a three year stint as a structural/design engineer, he worked for a small engineering firm (Vision Point Systems) in Fairfax, VA where he provided support within the Naval Research Laboratory’s (NRL) Center of Corrosion Science and Engineering. A new mechanical engineering program at George Mason University (GMU) was created in 2014 and in 2016, Erik accepted a position as an assistant professor where he taught classes in mechanical design, materials science, fatigue and fracture mechanics, statics, and mechanics of materials. In the Fall of 2022, Erik moved out West to Boulder Colorado where he serves as an associate teaching professor in the Ann and H.J. Smead Aerospace Engineering Sciences department.
